When I wrote my first novel, I just made one huge Word document. I do the same thing, basically, with my academic work (although usually I have 4-5 different Word docs with notes and stuff.
My second novel is just in a note-taking stage now (and probably will be until I finish my dissertation), and I've given Scrivener a try. It's working pretty well, but I'll know for sure when it gets much further along.

Scrivener is pretty good, though I still do way to much in a bunch of randomly named Word docs that I then have to somehow pull together during revision.
