Is there an in continuity reason why Michael and Walt weren't at the church? Are we just to assume that they weren't ready to move on. Nor were any of the 40+ survivors that were not the lead characters? or Danielle, or Alex, or Arzt, or Richard, or Miles ... why didn't anyone wake up Miles?

For Michael, it's the same reason Ben didn't go inside. Still had shit to deal with / atone for before heading to the final phase of the afterlife.
I believe it was suggested that you're in purgatory with those you were closest with or bonded to while living. The others you mention would've had their own groups.
Walt's got his own group of magical bird murderers.
After my week out and about traveling around, I can't tell y'all how fucking excited I am to get back to rewatching LOST. We're about half way through S3. I love this show so much!
