Pearl Jam is a good answer but even they're biggest change(No Code) has minimal songwriting contributions from the two guys who wrote nearly all of the first album.

Most of these are terrible answers. Even Radiohead barely qualifies. Bands like Metallica and Pearl Jam certainly evolved over the decades, and sure there first and last releases are going to sound different on many levels. But they're still operating within the confines of what they've always been.
Only a few examples here are fundamental, intentional shifts from what they started as. (Fleetwood Mac, Beastie Boys, and Tegan and Sara are the best examples, IMO)
